Create an incident form using a template from service portal

The Forms editor is easy to use. Your form takes up the entire screen, with enough space for a title and summary before the form fields. To edit or add a query to a form sector, click it. Choose the field form from the dropdown box next to the field, such as multiple choice, checkboxes, short answer, and so on.
Each field has a copy button that can be used to repeat the field, making it easy to add similar questions to your form. On the right side, there’s a delete button, options to make the field required, and a menu with additional options. You can change question types at any time, but switching from multiple choice, checkbox, or menu to any of the other question types will reset your field settings and questions. And, to quickly fill in fields, simply press enter to begin adding a new one.
Description and title: The title and description fields are applied to any type and field automatically (though the description is hidden by default on most fields), and the Tt button can be used to add an extra title block anywhere. On questions, you can leave the title and description blank, but the title of the main form must be filled in.
